How Crothall Cares for You

Crothall considers our associates to be its most valuable resource, and we believe you and your family should have access to high quality benefit options. Therefore Crothall offers up-to-date, flexible benefits at competitive cost. Through our benefits package you will select from an array of choices to provide maximum protection for you and your family, including alternatives that can help you save money on your taxes. Some benefit costs are shared between you and Crothall, while others are made available to you at reasonable group rates.

Eligibility for Benefits Coverage

Hourly Associates are eligible for benefits on the first day of the month following three (3) months of employment. Management Associates are eligible for benefits on the first day of the month following one (1) month of employment. Note: Bargaining Unit employees - effective dates may vary as they are determined by the union contract (CBA). Those employees (non-union) scheduled to work 30 hours or more per week on a consistent basis, are eligible for Crothall Benefits.

Benefit Highlights

Medical
Crothall offers several medical benefit plan - options. These benefits will vary depending upon on the State you reside in. Associate contributions are in general withheld on a pre-tax basis. Most medical plans also have comprehensive prescription drug coverage. Refer to your carrier’s benefit detail to fully understand the plan and prescription drug coverage.

Dental
Crothall offers two dental plan options (High/Low) through Guardian so that you can choose the plan that best meets your needs. Deductions are in general withheld on a pre-tax basis. Choose a Dental provider at www.glic.com or call 1-800-541-7846.

• Preventive Exams covered up to 90%
• Basic services are covered up to 80%
• Major services are covered up to 50% with calendar max up to $1500
• Orthodontist services are available thru the High option plan Vision

Crothall offers a comprehensive vision plan through VSP. Deductions are in general withheld on a pre-tax basis. For ease of use, no VSP ID cards or claim forms are needed. Choose a VSP doctor at vsp.com or call 1-800-877-7195

• Annual Eye exams
• Frame coverage up to $120 every 12 months
• In lieu of glasses, contacts are covered up to $120 every 12 months

Flexible Spending Accounts
Two “pre-tax” spending accounts are available:

• Medical Spending Account ($3,500 limit)
• Dependent Spending Account ($5,000 limit) Employee Assistance Program The EAP is offered at no cost to our Associates. This program provides both access and references to external resources for assistance with family matters, legal issues and work life solutions. Voluntary Disability Coverage
Disability coverage is available to eligible associates based on their employment status, EXCEPT, whereby a state disability plan exists (California, New York, New Jersey, Rhode Island, Hawaii and Puerto Rico). Bargaining Units disability coverage may also vary as it depends upon the established CBA.

Life & Accident Insurance
Basis life insurance coverage is provided to eligible associates at no cost. Supplemental Life and Accidental Insurance is also available should you want additional coverage. Voluntary Life Insurance Voluntary Life Insurance coverage is available for your spouse, domestic partner and/or your dependent children.

401(k) Plan
The quarter following six months of employment, eligible associates can participate in Crothall’s 401k plan.

• Pre-tax contributions can range from 1% - 25% of salary
• The company matches 35 cents on the dollar up to the first 3% of an employee deferral contributions
• A participant is 100% vested in the matching contributions upon being tenured with 3 years of service Employee Discounts

Dependent Coverage
You may include dependents on your healthcare coverage. Dependents are defined as a:

• Lawful Spouse
• Certified Domestic Partner who has been living with you for six months and is able to meet all eligibility criteria. (Based upon state guidelines.)
• Unmarried Child (including stepchild/child of your domestic partner) up to age 19.
• Unmarried Child up to age 23, if that child is a full-time student.
• Unmarried Child of any age who is mentally or physically unable to care for herself/himself.
